New Patient Paperwork
If you are a new patient scheduling your first visit with Southeastern Podiatry, we kindly ask that you complete our new patient paperwork before your appointment. This paperwork includes essential information about your medical history, current symptoms, medications/supplement list, and insurance details. Completing the new patient paperwork in advance allows us to make your check-in process smooth and efficient. Here is everything you need to know as a new patient at Southeastern Podiatry.
We use a Patient Portal that is used to input all of necessary information needed in order to be seen on your initial visit. Please contact the office at 850-942-0096 to get a link emailed to you. This link will allow you to complete all of your forms online to save you time with your scheduled appointment.
If you choose to complete your paperwork at the office on the day of the visit, you will need to arrive 20-30 minutes early. We will provide you an iPad Kiosk that you will use to complete your paperwork.
Please remember to bring a valid photo ID, your insurance card, medication/supplement list and any relevant medical records to your appointment.
